Aortic valve disease
October 2, 2019
In this systemic review and meta-analysis, the authors compared outcomes of valve-sparing root replacement vs. the Bentall procedure in patients with acute aortic dissection type A. Valve-sparing root replacement was associated with lower risk of early and late mortality compared with the Bentall procedure.

Open distal anastomosis in the frozen elephant trunk operation is usually performed in aortic arch zone 2 or 3. The authors compared outcomes in patients who underwent zone 2 versus 3 open distal anastomosis. They found that proximallization of the anastomosis simplifies the arch replacement, reducing the visceral ischaemia time.

Thomas Modine of the Regional University Hospital of Lille in France presents a discussion and demonstration of his modified valve sparing technique for aortic valve replacement that employs the 3f Enable valve.

Concomitant aortic root enlargement at the time of surgical aortic valve replacement is an option to avoid patient-prosthesis mismatch. In this single-center study, 4,210 patients underwent aortic valve replacement, of which 171 had concomitant aortic root enlargement.

This video demonstrates a ministernotomy for aortic valve replacement.

Henmi and colleagues analyzed the results of different reimplantation techniques of segmental arteries during thoracoabdominal aortic aneurysm repair. Among 172 patients, 111 underwent segmental artery reconstruction via graft interposition, 38 via single-cuff anastomosis, and 23 via island reconstruction.

E. Murat Tuzcu of Cleveland Clinic Abu Dhabi in the United Arab Emirates and Ourania Preventza of Baylor College of Medicine in Houston, Texas, discuss valve-in-valve transcatheter aortic valve replacement versus redo surgical aortic valve replacement.
August 29, 2019
Using an approach that minimizes valve depth relative to the membranous septum, the authors reduced the need for permanent pacemaker placement to a reliable and predictable 3% compared to the accepted standard of 10%.
August 21, 2019
The authors demonstrate a technique for aortic valve repair, which is durable, simple, reproducible, and economical, and awaits longer term results.
