This video describes the authors’ surgical technique to treat severe symptomatic hypertrophic obstructive cardiomyopathy through a median sternotomy. This technique is mainly inspired from the Morrow septal myectomy. The author will highlight the main anatomical landmarks that drive the surgical resection and that must be kept in the surgeon’s mind to allow for a safe and efficient surgical procedure. Finally, the video will discuss when it is advisable to add an edge-to-edge mitral stich during the modified Morrow myectomy and the author’s way of doing so.

Complex composite grafting can occasionally be required for total arterial revascularization in patients who need multiple grafts, so it is important for surgeons to learn as many techniques as possible. This video shows how to use double Y grafting utilizing bilateral IMA and a radial artery with endarterectomy to increase a patient’s chance at long-term survival.

One of the main worries with aortic valve-in-valve procedures (ViV) is coronary occlusion. This is a devastating complication with a greater than 50 percent mortality rate. This video describes a surgical alternative to the interventional Basilica procedure in a high-risk patient for ViV because of a very low coronary implantation.

This interesting video demonstrates a redo David I procedure for a thirty-year-old woman diagnosed with aortic insufficiency with bicuspid aortic valve. This case has the added layer of the patient also having had a PTA repair and reconstruction of right ventricular outflow track with a homograft when she was six.
