This article presents a tier-based framework with proposed benchmarks for the development of quality pediatric and congenital heart disease care in resource-limited settings. This was achieved by identifying the most common barriers to care and using a literature review method to determine the most effective solutions.

This single institution study compared outcomes of children <5 kg with univentricular (23) to those with biventricular (13) circulation supported by the Berlin Heart pulsatile VAD as bridge to transplant. Pts with biventricular flow had better success in getting transplant than those with univentricular physiology (92% vs 61%). 1-year survival after VAD insertion was better in the biventricular group (85% vs 50%).

This study reviewed records to establish how primary complete repair of pulmonary atresia with ventricular septal defect (PA-VSD) compares with staged repair. It was found that the survival rates of both methods are high, although the use of smaller shunts has resulted in earlier complete repair and is associated with increased risk of morbidity.
