This video showcases a redo mitral valve replacement performed at the Houston Methodist Hospital. The patient had previously undergone a valve replacement and a valve-in-valve transcatheter mitral valve replacement (TMVR) and now presented with severe mitral stenosis. The patient understood the risks of the operation and requested that the surgeons proceed.  

A standard redo sternotomy was performed. The heart was dissected out, and aortobicaval cannulation was performed. A transseptal approach was elected in this case. Once good exposure was achieved, the prior valves were resected, and a new valve was implanted.  

The aorta was opened to inspect the aortic valve and ensure that the leaflets were not injured.  Two stitches in that part of the ring were redone to alleviate restriction. The aortic valve was then moving normally. The valve was implanted, the aorta was closed, and the patient weaned from bypass. The postoperative course was unremarkable.
