Case of the Month: Noisy Breathing

Each month, thoracic surgeon and CTSNet Senior Editor Leanne Ashrafian spotlights unique surgical cases in cardiothoracic surgery through this Case of the Month series. For more videos in this series, click here.  

In this episode of the Case of the Month, thoracic surgeon and CTSNet Senior Editor Leanne Ashrafian is joined by Dr. John Pilling, a consultant thoracic surgeon at Guy's and St. Thomas' NHS Foundation Trust. They present part 1 of the case “Noisy Breathing.” Dr. Ashrafian provides an overview of the patient’s medical history and discusses the symptoms he was experiencing. Dr. Pilling then reviews the findings from the lung function tests, including an exploration of the results of the forced expiratory volume in one second (FEV1) and forced vital capacity (FV).  

In part 2 of this case study, thoracic surgeon and CTSNet Senior Editor Leanne Ashrafian and Dr. John Pilling, a consultant thoracic surgeon at Guy's and St. Thomas' NHS Foundation Trust, discuss the next steps that were taken. They explored the findings from the computed tomography (CT) scan and the various techniques and surgical solutions that were considered. They also examined the lesions found within the trachea, along with bronchoscopy images and details of endobronchial resection, including rigid bronchoscopy and cryotherapy. Additionally, they highlight the postoperative follow-up, specifying the duration of monitoring, and recommended actions. Finally, they discuss the rare tumor that was identified, explaining its characteristics, symptoms, and relevant facts.
